"Don't know why the Facility took the liberty to change my Doctor's appt. I was not told that the appointment had been changed. I found out about the change when my son went to confirm that I wld be taken by ambulance to my appointment."

"Our family member has been at the Nazareth Living Care Center for 2 ½ years and we have seen a change in directors four times (4x).
